Job Summary
Riverside Health Partnership have an exciting opportunity for a motivated, enthusiastic and dedicated Clinical Pharmacist to lead an excellent team of pharmacists, pharmacy technicians and medicine coordinators. This role will be an integral part of the multi-disciplinary team at Riverside Health Partnership, driving positive changes in healthcare for local people. We are looking for a professional who has a prescribing qualification and has proven management experience with skills to lead a team.
Main duties
* To work as part of the management team in the practice and support with achieving efficient and effective prescribing activities.
* To promote high quality, cost effective and evidence-based use of medicines within primary care working alongside the Nottinghamshire Integrated Care Partnership.
* To provide leadership to the pharmacy and medicines management team including mentoring, support and professional development.
* Working as part of a multi-disciplinary team in a patient-facing role, to provide primary support to general practice staff with regards to prescription and medication queries.
* Ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes.

Job Responsibilities
* Provide leadership to the pharmacy and medicines management team including mentoring, support and professional development.
* Develop effective protocols to ensure efficient and effective processes for patients.
* Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using medicines knowledge for specific disease areas.
* Take responsibility for the care management of patients with chronic diseases and undertake structured medication reviews to proactively manage polypharmacy.
* Ensure appropriate monitoring tests are in place when required.
* Provide specialist expertise in the use of medicines while addressing public health and social care needs to tackle inequalities.
* Lead on medicines optimisation and quality improvement, contributing to the quality and outcomes framework and enhanced services.
* Support patients to take medications effectively, reduce waste and promote self-care through structured medication reviews.
* Promote integration with wider healthcare teams to improve patient outcomes and manage workload.
* Assist in identifying cost-effective and evidence-based use of medicines.
* Reconcile hospital discharge letters and maintain accurate data in the clinical system.
* Participate in prescribing audits and implement changes with the practice team.
* Answer medicine-related enquiries from GPs, staff and patients and monitor outcomes of changes.
* Analyse medicines data to highlight risks and support decision making across the primary care team.
* Contribute to development and implementation of new services with medicinal components.
* Address medicines changes following MHRA alerts and national guidance.
* Work with the practice team to ensure compliance with CQC standards where medicines are involved, including audits.
* Contribute to PCN delivery of high quality standards and maintain patient confidentiality in line with data protection principles.
* Keep up to date with current pharmacy practice and attend required meetings.
* Represent the practice at locality Prescribing Leads Meetings and develop relationships with other pharmacy professionals across PCN and wider health and social care system.
* Respond to patient complaints in accordance with the practice complaints procedure and adhere to the General Pharmaceutical Council code of ethics.
Management duties
* Provide management and leadership to the pharmacy and medicines management team including mentoring, support and professional development.
* Promote a healthy, safe and effective working environment and ensure a robust 5-day service with adequate cover.
* Proactively develop the team and work with the management team to achieve the practice’s strategic aims.
* Arrange regular audits of work and manage own time effectively.
* Coordinate annual staff appraisals and arrange regular team meetings.
* Be involved in recruitment and keep the team up to date in all areas of training.
Qualifications
Essential
* Masters degree in pharmacy or equivalent
* Postgraduate clinical qualification and/or significant relevant clinical experience
* Registered with the General Pharmaceutical Council
* Qualified prescriber
* Minimum of 2 years experience as a pharmacist in relevant settings
* Experience of managing a team
* Experience of working with the public and acting responsibly within professional boundaries
* Experience in planning, developing and delivering pharmacy services
* Excellent knowledge of PCNs, General Practice and evidence-based medicine
* Ability to analyse and evaluate medicines-related information
* Strong time management and workload management skills
* Ability to communicate complex medicines information to patients, carers and healthcare teams
Desirable
* CPPE Clinical Pharmacist 18-month training pathway
* Experience working within primary care
