We deliver holistic care to patients within their own homes and residential homes, meeting the increasing healthcare complexities of our patient population. Assessment, planning, and delivering high standards of care are achieved through effective collaboration and partnership working within our health and social care integrated centre at Newbridge. The centre houses Redwood Community Hospital, social services, the district nursing team, GP surgeries, and many other health and social care professionals, enabling a collaborative approach to timely and effective community care.
