Hospice Community Clinical Nurse Specialist
StElizabeth Hospice is an independent charity and hospice, which improves lifefor people in the East Suffolk, Great Yarmouth and Waveney areas living with aprogressive or life-limiting illness. Ourmost recent Care Quality Commission report rated the hospice as Outstanding.
Establishedin 1989, the hospice has built a reputation for delivering high-level care topatients and the community it serves. Through medicine and therapy, they easepain; give life purpose and make life liveable. Last year the charity supportedover 4,000 patients and their families.
Thehospices work is centred on the individual's needs and they provide specialistsupport wherever it is needed; whether that is in the home, another setting inthe community, in hospital or in the hospice itself. Support is provided topatients and their families via the In-Patient Unit, the Community Care Unitincluding outpatients clinics, home visits from the Community Nursing, Medicaland Therapy Teams, the OneCall 24/7 telephone helpline and bereavement support.

We are offering an opportunity to be part of the CommunitySpecialist Palliative Care Team influencing the holistic care of patients andcarers with end of life and specialist palliative care needs. This is a chance to make a real difference topeoples lives.
We are looking for experienced Registered Nurses tojoin our Community Team on a permanent basis.You will work with our Community Teamproviding specialist palliative care advice and support to patients in the EastSuffolk area. You will work closely with your Team Leader andmulti-disciplinary team to provide face to face and telephone support, for acaseload of patients and participate in answering calls from our 24/7 adviceline.
Please note that we haveaflexible workforce policy which means that employees will be deployed in otherareas when there is the need to do so. This benefits service users andemployees.

To provide a rapidresponse service for patients needing urgent specialist care. Providing care asneeded such as drugs, position changes and interventions, if appropriate toimprove the comfort of the patient, or advising others to do so

To keep up to date withbest practise, disseminate and embed the learning into patient care.
Lead the development ofclinical standards, research and clinical audit, working within the ClinicalGovernance Framework of the Hospice and in palliative care in general.
To record all equipmentloaned out, ensuring records are kept up to date and that equipment isreturned.
To work autonomouslymaking decisions with minimal support but seeking advice from colleagues or/andsenior clinical team, as needed.

Experience
* Minimum 3 years experience in specialist palliative care at Band 6 level
* Experienced in providing palliative advice and support to end of life patients, families and other professionals
* Experienced in assessing patient needs that may be complex and subject to change
* Knowledge of the wider palliative care agenda
* Good knowledge of community care
* Working in the community and in the patients own home
* Have an understanding of safeguarding principles and be able to apply these within everyday practice and intervention/within your role.
