Job Summary
An exciting secondment opportunity has arisen for a band 4 Regional Renal flow coordinator based at the Royal Liverpool Hospital. The successful candidate will work closely with all members of the MDT to coordinate patient flow throughout the regional units and acute hubs, including HD coordination for inpatients, and will support the Renal Assessment Unit.
Responsibilities
* Produce and maintain a list of all new patients diagnosed in the Trust using sources such as IPM, CRIS, ICE, and liaise with departments such as Radiology and Pathology.
* Plan, coordinate and administrate MDT meetings for patients, maintaining patient lists, collecting case notes, films and results, and taking minutes.
* Collate and process routine data weekly to deliver timely and accurate information on waiting times and patient pathways to the Trust, Commissioners, Collaboratives and the Department of Health.
* Track patients through their pathway from referral to treatment and ensure all tracking is kept up to date daily.
* Follow up on diagnostic testing and ensure staff are aware of the patient pathway.
* Ensure patient is seen within appropriate timescales to meet national standards, liaising with clinicians, nurses and administration staff and highlighting potential breaches at MDT meetings.
* Maintain and provide available casenotes and relevant paperwork for MDT meetings.
* Send referral information to other trusts or services where relevant.
* Make appointments and provide information to patients about where to seek advice.
* Assist with general clerical duties including filing, photocopying and collating documents.
* Cover additional MDT meetings and tracking for other areas as required.
* Contribute to surveys, audits and the development of MDT operational policies.
* Support clinical teams in developing clinically useful, accurate and complete datasets.
* Work closely with Information Services and clinical areas to monitor waiting times and advise managers of any changes.
* Track risks and ensure compliance with the Data Protection Act and equality and diversity principles.
* Be flexible in working hours to meet the needs of the role.
* Any other duties required within the context of the grade.
Requirements
* Recent renal experience at band 3 is essential.
* Ability to understand medical terminology and clinical coding and willingness to undertake training as required.
* Strong organisational and communication skills, with the ability to work to deadlines and manage a varied workload.
* Experience working with multidisciplinary teams and coordinating patient flow.
* Proficiency with hospital information systems and a commitment to maintaining data quality and security.
* Respect for equality and diversity, ensuring colleagues’ and patients’ rights are not infringed.
* Compliance with the Data Protection Act and other relevant regulations.
