Job overview

An exciting 6-month secondment opportunity has arisen for a highly motivated and flexible individual to work within the Tissue Viability Service.

The post holder will be required to assist with cascading of knowledge in relation to all aspects of wound care, including pressure ulcers, leg ulcers, surgical wounds by way of formal and informal education sessions and one to one training.

We require a nurse with good communication and leadership skills who can facilitate change and development. Evidence of professional development is also essential.

The ability to be mobile across Wigan Borough is essential. This role will require you to work across all areas within the Trust.

This vacancy is open to all internal candidates. NHS Employees should ideally secure their manager’s agreement for a secondment arrangement ideally prior to applying, as this will be a condition of any offer made.

This vacancy is advertised on a fixed term basis for 6 months as it is funded on a non-recurrent basis. Whilst this post is advertised on a fixed term / secondment basis there is the potential that this post may become permanent in the future.

Main duties of the job

The post holder will be a specialist clinician within the field of tissue viability, facilitating the day-to-day delivery of a comprehensive Tissue Viability Service within both the community and acute settings by:

Providing specialist advice and education to all health care professionals/ carers throughout the borough to ensure high standards of clinical practice.

Acting as an autonomous practitioner within the trust assisting the Tissue Viability Specialist Nurses in developing the service.

Participating and assisting in research and audit as required. Implementing improving working lives initiatives as appropriate.

Contributing to the efficiency and effectiveness of the Tissue Viability Service.

Detailed job description and main responsibilities

Planning and Organisational Duties

1. To maintain accurate, comprehensive assessment and treatment records in line with legal and departmental requirements. This will include both electronic and paper records.
2. Plan own workload to maximise effective use of resources
3. Manage referrals into the service in a proficient manner, triaging and prioritising service provision based on individual needs and refer on as appropriate
4. To effectively manage change within a range of care environments
5. To work with medical and Allied Health Professional colleagues to ensure, where appropriate, that services are co-ordinated and combined, including joint assessments
6. To take part in audit to ensure that standards of practice are monitored and maintained
7. To record and submit professional and clinical activity
8. To participate in the development of clinical standards, protocols and formularies
9. Manage working hours flexibly within each day to ensure the needs of the service are met giving priority to patient care and able to respond to any changing needs
10. Maintain membership at the appropriate forums and organisations both locally, regionally and nationally to keep abreast of new developments within tissue viability

Communications and Key Working Relationships

11. To have highly developed communication skills e.g. empathy, reassurance, motivation, negotiation, persuasion
12. To have highly developed interpersonal skills in order to deal with sensitive and potentially conflicting situations
13. To address barriers to communication within the Service, utilising the expertise of other agencies e.g. translation service
14. To promote patient involvement in Tissue Viability Service
15. To develop strong links with the wound care link practitioners to support network development

Responsibility for Finance

16. Assist the Tissue Viability Specialist Nurse in wound care product evaluations
17. Effective prescribing of wound care products

Responsibility for Human Resources

18. Assist the Tissue Viability Specialist Nurse in delivering formal training sessions to all levels of staff in relation to tissue viability
19. Provide one to one education to all levels of staff in relation to tissue viability when conducting joint patient reviews
20. Contribute to and promote a culture that values staff, recognising and acknowledging achievement whilst positively addressing poor practice
21. Be involved in annual, individual performance reviews and personal development plans

Responsibility for Teaching

22. Assist the Tissue Viability Specialist Nurse in delivering formal training sessions to all levels of staff in relation to tissue viability
23. Provide one to one education to all levels of staff in relation to tissue viability when conducting joint patient reviews
24. Assist the Tissue Viability Specialist Nurse in implementing evidence-based policies, procedures and guidelines and support improvements in practice
25. Maintain personal and professional knowledge and understanding of current issues through commitment to self -development through life-long learning
26. Attend and complete mandatory training to comply with Trust objectives

Person specification

Qualifications

Essential criteria

27. NMC Registered Nurse (adult branch)
28. Diploma in Nursing or Supporting portfolio of evidence.
29. Evidence of recent study.

Desirable criteria

30. Nurse prescribing
31. Teaching Qualification
32. Tissue Viability course at degree level
33. Leg Ulcer Management course at degree level or equivalent

Experience

Essential criteria

34. Experienced in leg ulcer management.
35. Experienced in complex clinical assessment.
36. Experienced in working autonomously
37. Experienced in working in primary and secondary care.
38. Experienced in multi-disciplinary team working
39. Experienced in wound care management.

Desirable criteria

40. Experience in presenting formal and informal training sessions
41. Experience of working as a Wound Care Champion
