Key Responsibilities
* Patient Care: Responsible for delivery of practice nursing services, working as part of the practice multidisciplinary team.
* Administrative Support: Manage appointments, accurate maintenance of clinical records, effective use of SNOMED codes, and management of chronic disease registers; maintain records such as baby immunisations and vaccinations; efficiently manage phone inquiries and provide excellent patient service.
* Clinical Care: Identify, manage and support patients at risk of developing long‑term conditions to prevent adverse effects; routine nursing care according to clinical‑based evidence, NICE, and NSF guidelines; provide vaccinations/immunisations and travel medicine, wound care, request pathology services with subsequent result processing; conduct chronic disease clinics, deliver patient care and make referrals to secondary/specialist care when required; prioritise health issues, recognise and refer patients with mental health needs, and support patients in medication use; manage safeguarding matters, opportunistic health promotion and emergency drug kit management.
* Health Promotion: Support initiatives by providing information to patients on healthy lifestyle choices and encouraging preventive measures; contribute to patient education programmes.
* Infection Control: Adhere to protocols to maintain a safe and clean environment for patients and staff; assist in organising clinical areas.
* Team Collaboration: Collaborate with GPs, nurses and administrative staff to ensure smooth operations within the practice.
Requirements
* Registered Nurse qualification and registration with the Nursing and Midwifery Council with a valid NMC PIN.
* Excellent communication and people skills.
* Ability to work effectively in a demanding environment.
* Ability to work both independently and as part of a team.
* Strong organisational skills and attention to detail.
* Willingness to learn and adapt in a fast‑paced healthcare environment.
* Compassionate and patient‑centred approach to care.
