Overview

To drive participation and coproduction amongst the carers, families and friends, including the people and communities who use our services, to underpin and enable positive carer and family engagement in the delivery of safe and quality driven services now and for the future.

The post holder will develop opportunities, build skills, knowledge and confidence amongst carers, families and staff to increase participation in line with national best practice, coproduction principles and underpinned by the Triangle of Care standards and the Patient Carer Race Equality Framework (PCREF).

The post holder will champion the rights and needs of carers and families, ensure there is consistent and concise information available and communicated to them across all the Care Groups. This role will lead in developing and maintaining positive collaborations with Carer and Families support networks and groups within the voluntary and community sector aligned with the geography of the Trust’s activities. Ensuring opportunities for collaboration to ensure the best access and support for the carers and families of the patients and service users accessing the Trust's services. You will be working across the Trust’s three Care Groups of Mental Health, Community and Forensic services, ensuring services listen and respond to carer/family and patient feedback and views, taking a lead role in ensuring services work in partnership with carers/families through embedding coproduction principles to drive improvements in services, care outcomes and communications.

You will be responsible for delivering the development of the approach for the Triangle of Care Self-Assessment processes and supporting ongoing action planning, ensuring carers and families’ involvement and contribution.

You will develop effective working relationships with a wide range of carers, families, patients, staff and carers and family support organisations within the Trust and across the communities we work with in Nottingham/shire.

Responsible for ensuring information for Carer & Family on their rights and pathways for support are up to date, regularly reviewed and reflected and used across a range of communication pathways.

Notes

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
