Job responsibilities Please see attached Job Description for full details Specific functions will be to: To work within the community as an autonomous, accountable ACP in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services. Provide advanced clinical assessment of patients including conducting appropriatediagnostics, and as appropriate provide definitive treatment, develop a clear clinical management plan, or make necessary referrals to other members of the multi-disciplinaryteam. Undertake physical examination as indicated by the patients condition. Perform clinical procedures appropriate to the history and physical examination of thepatient. Request, interpret and act on investigations. Advise patients on general healthcare and minor ailments and promote self-managementwhere appropriate, including signposting patients to other community or voluntaryservices. Where appropriate, provide definitive treatment, including prescribing medications following policy, patient group directives, NICE and local guidelines and local carepathways. Manage undifferentiated undiagnosed conditions, identify red flags and underlying seriouspathology and take appropriate action. Use complex decision making to inform the diagnosis, investigation, and completemanagement of episodes of care within a broad scope of practice. Take a personalised care approach and population-centred care approach to enable shareddecision making with the presenting person. Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing) Support the monitoring of patients with management of long-term conditions and multiplecomorbidities. To carry out visits to patients private homes, as well as visits to patients in Nursing / Carehomes. To act as an ambassador for acute admission avoidance where safe and appropriate. Assist with the annual flu immunisation programme. Provide an alternative model to urgent and same day GP home visit for the network. Participate in the audit process, evaluation and implementing plans and practice change inorder to meet patient need. Communicate at all levels across organisations ensuring that an effective, patient-centredservice is delivered. Communicate sensitive and complex information to patients, relatives and carers to be able explore complex issues relating to care. Communicate proactively and effectively with all colleagues across the multi-disciplinaryteam, attending and contributing to meetings as required. Maintain accurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated with patienthealth care and registration with the practice. Analyse reports in order to support patient care. Ensure the provision of high standard of care at all times. Work as a multidisciplinary, support and contribute to timely discharge planning whereappropriate. Contribute to the development of efficient pathways of care.
