We are seeking an experienced and highly motivated Band 6 Children’s Community Nurse to join our dynamic community nursing team. The post holder will provide specialist, child and family-centred nursing care to children and young people with acute, chronic, and complex health needs within their own homes, schools, and other community settings.
As a Band 6 CCN, you will work autonomously, managing a defined caseload and coordinating care in collaboration with children, families, and the multidisciplinary team. You will demonstrate advanced clinical skills, sound professional judgement, and a commitment to delivering safe, effective, and evidence-based care closer to home.
The role includes providing clinical leadership, supporting and supervising junior staff, students, and support workers, and contributing to service development, quality improvement, and clinical governance activities. You will play an active role in safeguarding, health promotion, and supporting families to develop confidence in managing their child’s health needs.
Main duties of the job
1. Provide high-quality care to children and young people in their homes, schools, and community settings
2. Assess, plan, implement, and evaluate individualised care plans for children with acute, chronic, and complex health needs
3. Deliver a range of clinical interventions, including medication administration, wound care, enteral feeding, and other specialist procedures as required such as accessing CVADS.
4. Work autonomously while managing a caseload, prioritising workload effectively and responding to changing clinical needs
5. Liaise closely with multidisciplinary teams, including GPs, consultants, therapists, schools, and social care services
6. Contribute to safeguarding children and young people, recognising and escalating concerns in line with local and national policy
7. Maintain accurate, timely, and confidential clinical records in line with professional and organisational standards
8. Support and supervise students, junior staff, and support workers as appropriate

The Community Children's Nurse is responsible for providing skilled nursing care for sick children in their own homes or other suitable venues that would otherwise be performed in an acute hospital setting. They are also responsible for the effective assessing, planning implementation, delivery and evaluation of individualised nursing care to children as required and under the direction of the Team Lead. The Nurse is expected to work with minimal supervision and will be designated to take charge and work as a lone worker within the community on a regular basis.
The Nurse will provide care at home without supervision to facilitate the prevention of admission or reduction in hospitalisation.
