An excellent opportunity has arisen for an enthusiastic, experienced Care Navigator to join our friendly practice team with 5 partners and 8,400 patients. The post is for 37 hours per week, with a requirement to remain flexible to cover colleagues who may be sick or on holiday.
Main duties of the job
To undertake a wide range of reception, secretarial, and administrative duties, providing support to the multidisciplinary team. Duties include greeting patients, managing appointment systems, processing information, and acting as the central point of contact for patients and external agencies, including the use of the electronic referral service (ERS).

Job responsibilities
* Booking appointments and recalls according to practice policy
* Familiarity with all appointment systems in use
* Processing appointment requests via phone and electronic requests
* Signposting patients to appropriate services
* Routing patients correctly during appointments
* Managing urgent patient consultations
* Responding to patient and visitor queries
* Signing in visitors and informing staff
* Explaining practice procedures to new and temporary patients
* Data entry for new and temporary patient registrations
* Requesting and referring home visits
* Handling lab results and patient queries
* Managing incoming calls and messages
* Passing on unresolved issues to the next shift
* Maintaining medical records and correspondence
* Updating patient details
* Checking and securing consulting rooms
* Opening and closing the premises daily
* Reporting issues to team leaders
* Ensuring adequate supplies of forms and tidiness of the waiting room
* Processing referral letters, including urgent 2-week wait referrals
* Managing electronic referrals and workflow
* Preparing outgoing post
* Supporting clinicians with referrals and related tasks
Adhering to confidentiality, using judgment, and supporting clinical staff. Managing private charges, photocopying, data input, scanning documents, liaising with external agencies, raising invoices, and system searches are also key duties.
