Overview
We have a 37.5 hour Band 4 Community Nurse Associate post in the Poole North Locality District Nursing teams. You will deliver planned care to patients in the community setting. This role supports individual development and training needs, works effectively within a large team of mixed skills for complex patients, and relies on effective communication and multi‑disciplinary working.
Responsibilities
* Provide high quality, person‑centred care within your sphere of practice, always considering patients’ privacy, dignity and safety.
* Deliver planned care to a group of patients in line with Nursing and Midwifery Council (NMC) standards and Trust policies and protocols.
* Demonstrate ability to work effectively as part of a team and provide support/guidance to less experienced staff and learners within the clinical environment as needed.
* Manage a defined group of patients/service users under indirect supervision of a Registered Nurse, undertaking delegated tasks independently using evidence‑based/client‑centred principles.
* Contribute to ongoing assessment, planning, implementation and monitoring of patient/service user care, escalating to a senior registered practitioner as appropriate.
* Promote health and prevent ill health in service users, contributing to integrated care.
* Take responsibility for planned/defined tasks as required.
* Plan and undertake clinical tasks guided by standard operating procedures and protocols within the limitations of the Registered Nursing Associate role.
* Undertake delegated clinical skills within your scope of practice after relevant training, maintaining competency and adhering to local and national guidance.
Requirements
* Enhanced Disclosure and Barring Service (DBS) check is required; the Trust will cover the cost. Applicants who subscribe to the DBS update service may present a valid certificate instead of a new check.
