What You Will Do
* Conduct person‑centred, strengths‑based assessments to promote independence and wellbeing.
* Develop and implement tailored care plans in collaboration with the Social Services Team.
* Maintain accurate and up‑to‑date electronic case records.
* Work flexibly to meet service user needs while prioritising safety and quality of care.
* Travel within the local area (car driver preferred) to complete community‑based visits.
About You
* Qualified Occupational Therapist, HCPC registered, maintaining professional standards and compliance.
* Experience within Adult Social Care settings.
* Strong assessment, communication, and partnership‑working skills.
