This role plays a vital part in delivering high-quality, person-centred support that aligns with the council’s commitment to continuously improving modern and competitive services for our community. In this role, you will work under the guidance of qualified practitioners, holding a caseload and carrying out assessments to develop tailored support plans. You will play an active role in risk management, monitoring outcomes, and ensuring individuals receive the most effective support. Our Service operates between 8am and 8pm, over 7 days a week, 365 days a year. Please note: Rota patterns will include alternate weekends (as part of a set rota) and Bank Holidays. Who we are looking for… We are looking for a compassionate and motivated individual with experience or awareness of social care, healthcare, or the voluntary sector. You should have a Level 3 qualification in Care (or be willing to work towards one) and strong communication skills to collaborate with customers, carers, and stakeholders. A proactive approach to risk management, moving and handling, and health and safety is essential, along with the ability to plan workloads, meet deadlines, and use IT effectively.
