Band 6 Static Physiotherapist- Rapid Response Team
This is an exciting job opportunity to work within the Rapid Response Team. This is an expanding service which provides support to patients both in the acute and community setting.We are building towards a locality hub model with prospective bases in Chester, Ellesmere Port and Tarporley.
This post is a permanent post within the Rapid Response Team which would be suitable for an experienced Physiotherapist who enjoys the challenge of supporting a highly varied and complex caseload of patients in their own home following their discharge from the acute hospital setting.
This post is to cover a 7-day service between 8am-9pm.
Main duties of the job
Main duties as outlined below:
The successful candidate will be based at one of the locality hubs but will work across all areas to support the Rapid Response Team therapy caseload. The entire patient discharge pathway will be covered within this role. From assessing and discharging patients in SDEC (Same Day Emergency Centre) and A& E, completing discharge visits in the Community within 2hours of hospital discharge to ascertain patient's needs, through to managing a large and varied therapy caseload within the community.
The successful candidate will need to have had experience in the core Physiotherapy rotations in addition to being confident and competent in discharge planning and autonomous working within the Community setting.
