Overview

We are looking to recruit an enthusiastic and dynamic Advanced Nurse Practitioner to join our growing multi-disciplinary primary care team. You will work alongside our multi-disciplinary clinical team of GPs, Pharmacists and Nurses to deliver high quality patient care. This permanent position is offered on a part-time basis worked over 3.5 days. A full day of clinic runs from 9:00 to 13:00 and from 14:00 to 18:00. There are paid tea breaks in each session in addition to any unpaid lunch break. We welcome applications from qualified Advanced Nurse Practitioners from all backgrounds; however you must have at least 2 years prescribing experience and 2 years experience working within an Advanced Nurse Practitioner role to apply.


Responsibilities

* As an Advanced Nurse Practitioner with independent prescribing, deliver face-to-face services and provide health information to patients from appropriate healthcare professionals. Provide safe and effective health assessments and treatment of patients within the establishment.
* Be accountable for your practice including assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention.
* Demonstrate safe clinical decision making and expert care, including assessment and diagnostic skills for patients within the service.
* Collaborate with the healthcare team to meet patient needs and support the delivery of policy and procedures; implement agreed management plans and follow approved protocols as appropriate.
* Provide a holistic clinical service with support and supervision from GPs.
* Maintain ongoing care planning and support, with emphasis on prevention and self-care, and address patients with socio-economic issues where applicable.
* Promote health and well-being through health education, screening and therapeutic communication to improve self-care and independence.
* Participate in screening for disease risk factors and early signs of disease; refer patients to other services or agencies as appropriate, after discussion with a GP.
* Encourage patients to engage in face-to-face consultations where clinically necessary; work with allied health professionals to ensure delivery of Quality and Outcomes Frameworks.
* Exercise professional judgment to manage risk, including complex and unpredictable events, and support teams to ensure safety for individuals, families and carers.
* Use enhanced communication, negotiation, and conflict management skills with empathy and reassurance to overcome barriers to acceptance.
* Work with the multi-disciplinary team within the Practice and across the wider primary care network to promote integrated and seamless pathways of care.
* Monitor the effectiveness of clinical practice through quality assurance strategies such as audits and peer review.


Person Specification


Qualifications

* NMC Registered
* Advanced Nurse Practitioner Qualification
* Independent nonmedical prescriber


Desirable

* MSc (Advanced Practice)


UK Registration

* Current UK professional registration is required.


Experience

* Essential: 2 years prescribing experience
* Essential: 2 years experience working as an Advanced Nurse Practitioner
* Desirable: Experience of working in primary care or General Practice


Disclosure and Barring Service Check
