What you will do:
* Conduct comprehensive assessments and reviews for individuals receiving care and support to ensure their needs are met effectively.
* Undertake care management and safeguarding duties in line with statutory requirements and Adult Social Care legislation.
* Develop strong relationships and work creatively with Partner Agencies to achieve the best outcomes for service users.
* Communicate clearly and work flexibly within a multidisciplinary framework, exhibiting excellent report writing and computer skills.
* Maintain accurate record keeping as an essential aspect of this role and ensure compliance with quality standards.
* Provide guidance and potentially supervise less experienced staff members, contributing to a collaborative and nurturing team environment.
