About the role
Are you a HCPC registered Speech & Language Therapist looking for an opportunity to work with an adult acquired caseload?
An opportunity has arisen to join our innovative service as a Community Speech & Language Therapist at The Mid Yorkshire Hospitals NHS Trust. The role is full time at 37.5 hours and it will involve working across the Wakefield and district community; visiting patients in Nursing and Care homes, private homes and in clinics. We work with a wide range of caseloads including stroke, brain injury, progressive neuro, dementia and respiratory providing a high quality service for our patients who have swallow and communication difficulties. You will work alongside skilled and supportive Community SLT colleagues.

The Adult Speech & Language Therapy Team at Mid Yorkshire is a very innovative service and we are focusing on high quality care for patients alongside optimising learning and career development opportunities for staff. We are a busy department and a team of dynamic, innovative and friendly skilled Speech and Language Therapists. There will be access to specialist level training, regular clinical supervision and CPD. Our service is able to offer AMPCARE, EMST and Videofluoroscopy assessment. We are also setting up a FEES service within the Adult SLT team.
In order to fulfil the role you will be able to demonstrate skills in the assessment, treatment and management of communication and swallowing difficulties. You will be supported by the Speech and Language Service Lead, Team Lead and Clinical Leads, and specialist speech and language therapy colleagues. You will carry your own clinical caseload and contribute towards audit and service development projects.

Main duties
The main duties of this role include:
The post holder will work flexibly as required to maintain a responsive, patient centred and evidence-based service for optimal patient outcomes.
You will work across a range of settings; including care homes, private households and out patient clinics. It is essential that you can travel around the Wakefield district area to fulfil the demands of the role.
If you are a newly qualified SLT or new to adult services, you will be provided with a supportive development plan.
You will manage a communication caseload and provide assessment, diagnosis and treatment for patients who have long term conditions or stroke, dementia, cancer or other conditions which cause communication impairments. This can include group therapy.
You will work closely with clinical lead for community and with other SLT colleagues in the community setting.
You will be a key part of the MDT; working with GP's, district nurses and community groups such as stroke association. You will also provide clear and supportive handovers to patients, families and carers when working through assessment and therapy tasks.
If you have competencies and skills in dysphagia management, you will be encouraged to support patients with swallow presentations.
There is office based planning and report writing time, plus ringfenced time for MAST and CPD, plus regular supervision. You will have access to an agile device.
You will liaise with the hospital SLT staff through shared pathways or at meetings / CPD sessions; enabling excellent networking and development opportunities. You will contribute to SLT service development groups to enhance the service.
You will be delivering training and education to patient, carers and third parties.
You will supervise, support and provide clinical education as required to students, and assistants.
You will lead on relevant projects or audits for the SLT service.
Excellent inter-personal, communication, motivational, and team working skills are important components of how we work, in addition to our clinical skills.
Key requirements
