An exciting opportunity has arisen within the Telford and Wrekin Community Mental Health Services for a Band 5 Occupational Therapist. This post includes a 12‑month preceptorship programme designed to support your development into a Band 6 role.
Key Responsibilities
* Conduct occupational therapy assessments to identify care needs, formulate individualised care plans, and deliver evidence‑based interventions aligned with the person’s goals.
* Collaborate within a multidisciplinary team, provide therapeutic input and support individuals in achieving agreed outcomes.
* Coordinate and/or participate in the review of progress, using recognised outcome measures and ensuring the involvement of service users and their identified carers throughout the clinical pathway.
* Manage a developing caseload of increasing complexity, undertaking specialist assessments, care planning, and delivering non‑pharmacological, evidence‑based interventions.
* Contribute to quality assurance through auditing and monitoring practices, while completing risk assessments and promoting positive risk‑taking to enhance independence and quality of life.
* Maintain an effective reporting system, observe and report verbally and in writing on patient conditions.
* Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care‑planning and evaluation process.
* Support the provision of formal and informal training for pre‑/post‑registration students, staff members, carers and service users.
* Participate in a duty rota/shift coordination system to ensure consistent service delivery across operational hours.
* Maintain accurate clinical records, uphold clinical governance standards and ensure care aligns with national and local guidelines.
Qualifications & Training
* Dip COT or Degree in Occupational Therapy.
* Health and Care Professions Council Registered Occupational Therapist.
* Member of RCOT.
* CPD folder evidence of ongoing learning and development.
Experience – Essential
* Experience of formulating assessments, planning, coordinating and supporting individuals experiencing difficulties.
* Experience of leading or contributing to primary key benchmarking (PKB)/care plans.
* Experience of integrating care delivery across multi‑disciplinary and multi‑agency teams.
Experience – Desirable
* Experience of working in a community setting and making decisions autonomously.
Skills, Knowledge & Abilities – Essential
* Knowledge of the RCOT Mental Health Strategy.
* Demonstrate understanding of equality and anti‑discriminatory practice.
* Knowledge and experience of occupational therapy models, interventions, specialist assessments and outcome measures.
Skills, Knowledge & Abilities – Desirable
* Range of specialist assessments and interventions to support occupational performance (e.g. ALLES, sensory assessment, VDTO, MOHOST, TFL’s, OT Ax).
* Knowledge of the wide range of statutory independent and third sector service provision for adults with mental ill health.
