As an Independence and Wellbeing Practitioner in the Woodbridge Integrated Team, you’ll work alongside colleagues, partners, and communities to build trust, share ideas, and deliver services that make a lasting impact. Your role and responsibilities As a key member of our integrated team, you’ll work with Social Care professionals, Occupational Therapists, Physiotherapists, Community Nurses, and GP Patient Coordinators to support individuals across Woodbridge. Your responsibilities will include: conducting Care Act assessments, reviews, and Mental Capacity Assessments using a person-centred, strengths-based approach signposting individuals to local services and community resources that promote independence and wellbeing developing short-term support plans and assessing long-term care needs across various settings including homes, extra care housing, and residential/nursing facilities providing duty cover – responding to calls, emails, and referrals, and reviewing contact records to ensure timely action safeguarding individuals by co-creating safety plans that reduce risk while promoting autonomy and choice. You will need solid understanding of the Care Act 2014 and Mental Capacity Act 2005 knowledge of Adult Social Care services and care management processes experience in person-centred, enabling approaches to maximise independence and wellbeing strong research, organisational, and prioritisation skills the ability to work collaboratively and share ideas with colleagues excellent communication and listening skills, with the ability to record information clearly emotional awareness and sensitivity in complex situations experience working with individuals with disabilities or age-related conditions. The team The team is made up of a range of professionals who all work hard to support the people of Woodbridge by providing long-term care plans, short term support, home visits, therapy, adaptations, advice, guidance and signposting. After an integrated team project in 2023, we developed the Woodbridge Holistic Assessment Team. This team of professionals run a monthly clinic to offer a range of assessments and advice to people who are experiencing falls or near misses and the associated conditions. In December 2024, we moved offices, which means that Health and Social Care are now on the same floor plate. We have Primary Care Network (PCN) sitting downstairs too, which has helped develop our relationships and integrated working further. We have a gym for therapists to assess mobility and provide equipment aids. Additionally, we have a leg ulcer clinic and non-housebound clinic supporting those with Picc lines and catheters, for patients that are able to access their local community; this reduces demand on the Community Nursing Team. We follow a hybrid model of working but have seen the benefits of more presence in the office as this a shared space with many professionals.
