We are looking for a registered Learning Disability Nurses to join our Community Learning Disability Teams across Manchester.
You will be working as part of a nursing led team with wider support from the multi-disciplinary community based team. The role will involve leading and contributing to the effective provision of services to people with a learning disability, complex needs and Autism.

Main duties of the job
To provide and manage a caseload of people who use the service using evidenced based and person centred principles to assess, plan, implement and evaluate interventions.
This will involve working within a multi disciplinary team with the emphasis on quality service delivery through holistic assessment and identification of needs.
To participate in project and group work, to enhance service delivery, in order to meet national and local targets.
To contribute to the planning, delivery and evaluation of both professional and service developments.

Qualifications
* Registration with the NMC: Registered nurse, learning disability (RNLD)
* Specialist community practitioner award, mentorship award
Experience
* Work in partnership with other professionals in a multi-disciplinary team
* Experience of working with people with learning disabilities
* Experience of assessment of need of people with learning disabilities
* Experience of working with people who challenge services
Skills
* Ability to manage own time
* Ability to participate in training individuals and groups
* Comply with professional supervision framework and supervise junior members of the nursing team
* Ability to participate in project work
Knowledge
* Evidence based practice
* Ability to understand the range of health needs of learning disabled adults
* Awareness of national guidelines and legislation relating to health and social care and the provision of services for learning disabled adults
