Overview of the Role
The Healthcare for Older People (HOP) team is dedicated and expanding, with consultant numbers increasing from 3 to 9.15 WTE over the past 15 years, including specialists such as Specialty Doctors, a Consultant Nurse, an Advanced Clinical Practitioner in Frailty, and Specialist Nurses in Parkinson’s Disease and Movement Disorder.
Our specialty services include two 28-bedded acute medical wards for older people, a 28-bedded acute stroke ward with 2.9 WTE Consultant Stroke Physicians, and a 15-bedded stroke rehabilitation ward at a community hospital. We also provide rehabilitation beds across four community hospitals, Intermediate Care beds in local care homes, Orthogeriatric, Parkinson’s Disease & Movement Disorder services, and rapid access clinics. Additionally, we oversee medical cover for 18 beds at Totnes Community Hospital, supporting training for medical students, foundation doctors, and registrars.
Key Responsibilities
1. Deliver high-quality Consultant-led care, providing clinical leadership and supervising junior medical staff to ensure the highest standards of care.
2. Maintain an interest in medicine for community-dwelling frail older adults, offering comprehensive geriatric assessments across various settings, including home, care homes, community hospitals, and intermediate care.
3. Ensure rapid assessment, treatment, and clinical decision-making for all patients, maintaining safety, quality, and performance standards.
4. Contribute to teaching and training at undergraduate and postgraduate levels, supervise junior staff, and support the department’s clinical governance initiatives.
5. Communicate effectively with clinicians and managers within and outside the department to address service issues.
6. Participate in audit and quality assurance processes.
