Join us for an exciting opportunity as a Qualified Social Worker in the Adults Transition Team based in the vibrant community of Bolton. Earn £30 hourly with this full-time locum role that provides a unique chance to support young people aged 14 to 25 with special educational needs and disabilities. With the initial four weeks full-time, this role will be reviewed for possible extension.

What you will do:
* Co-work with children services to ensure a smooth transition for young people from childhood to adulthood.
* Conduct holistic assessments and develop comprehensive transition plans aligning with the Care Act 2014.
* Collaborate with multidisciplinary teams to advocate and implement personalized support plans.
* Engage with young people and their families to provide guidance and advice on available pathways.
* Maintain accurate and up-to-date records, ensuring compliance with legal and procedural frameworks.
* Travel across Bolton as required, with the flexibility of utilizing your own vehicle with full business insurance.
This opportunity requires a qualified social worker with a minimum of two years post-qualification fieldwork experience. Familiarity with the Care Act 2014 and experience with disabled young individuals are essential. A full driving licence with access to your own vehicle is crucial.
