The Clinical Care Partner (RN) will work closely with clients, families, care teams, and external healthcare providers to ensure comprehensive and individualised care plans are developed and implemented effectively.
What You’ll Be Doing…
With the upcoming Support at Home reforms, now is the time to make a real impact on the lives of older Australians. If you're passionate about enhancing quality of life and shaping a strong clinical service, this role is for you.
As a key member of our team, you'll coordinate, monitor, and support the clinical care needs of clients receiving home care services. You'll also manage a caseload of Home Care Package clients and work closely with Care Partners to assess and address clinical needs.
Some Of Your Responsibilities Will Be
* Conducting regular reassessments to ensure that clients’ needs are being met effectively
* In collaboration with the Scheduling team, coordinate the delivery of services such as personal care, meal preparation, cleaning, and transportation
* Monitoring clients’ funding, taking corrective action to ensure services are provided within funding and supporting the management of unspent funds
* Adhering to Aged Care standards and guidelines, and organisational policies and procedures
* Conducting regular team meetings, performance reviews, and ongoing training to ensure high standard or care delivery
What You Will Need…
To be a successful candidate you will have:
* Registered Nurse of a minimum of 3+ year's experience in aged care – Home or residential aged care
* Ability to showcase comprehensive knowledge and practical experience in client care, clinical procedures and healthcare protocol
* Proficiency in an additional language, particularly Arabic, is desirable
* Experience in residential care and dementia care is desirable
* Experience in home care, case management, or a related clinical setting. At least 2 years of supervisory or leadership experience
* Strong leadership, communication, and interpersonal skills and ability to work collaboratively in a multidisciplinary team setting
