Clinical Lead Community Nursing

The closing date is 28 May 2026

We are looking to recruit an enthusiastic Clinical Lead to join Chester South Care Community Team. You will be joining a multidisciplinary team including but not limited to Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, Specialist Palliative Care Nurses, AHP's as well as many other specialist professionals who visit the team.

We are looking for someone who is enthusiastic, self‑motivated and who provides confident support and clinical leadership to clinicians alongside working in collaboration with other professionals. Has excellent communication skills and an ability to work with patients and their families/carers and when necessary, act to safeguard individuals who are at risk from harm and abuse.

This post is full time 37.5 hours per week.


Main duties of the job

* Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload
* Work collaboratively with the wider health, social care and the third sector partner agencies to meet the needs of patients
* Ensure patients receive the right care, by the right person at the right time and the right place
* Place the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, and an emphasis on people achieving independence whilst improving quality of care
* Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives
* Be aware of local public health community profiles and how this influences the required care for the population
* According to community profiles: understand the skills required from the Community Care Team
* Focus on the delivery of person centred care
* Provide effective, evidence based care to a defined population through case load management and monitoring of care standards
* Provide clinical leadership


Qualifications

* First level registered general adult nurse
* Evidence of teaching and assessing
* SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment


Knowledge and Expertise

* Proficiency in delivering high quality evidence based care
* To be able to demonstrate continuing professional development
* Ability to work and communicate effectively within an MDT setting


Experience

* Significant experience of working within a community healthcare setting
* Experience of making autonomous decisions
