The Rapid Transfer Service (RTS) is responsible for managing complex patient discharges from the acute hospital to community pathways, enabling patients' care needs to be assessed outside of hospital.
This post involves assessing end of life patients in the acute hospital who wish to return to their own home or to a care home. The role includes working within the acute Trust to assess patients’ needs for discharge.
The post holder will work as a key member of the RTS, aiming to ensure the timely, effective, appropriate, and safe discharge of end of life patients requiring supportive discharge from the acute hospital into a community setting.
The RTS service operates 7 days a week, covering weekends and bank holidays on a rota basis. The team is responsible for complex discharges across all three acute sites in east Kent, with occasional cross-site working to ensure service coverage.
This post does not involve delivering direct patient care. However, a clinical nursing background is essential.
Role Specific Responsibilities
1. Assessment, planning, implementation, and evaluation of holistic care linked to end of life needs.
2. Supporting the supervision and development of staff.
3. Actively working as a member of the integrated team to provide high-quality care to service users.
4. Undertaking a range of clinical and non-clinical interventions to support service outcomes.
5. Acting as lead and senior practitioner to support junior staff with assessments and re-assessments of service users with complex needs across relevant fields.
6. Supporting and advising senior managers to prioritize efforts, drive service improvements, and achieve business objectives.
