Main duties of the job
You will work within a newly developed MDT alongside colleagues from the Communities Directorate, Adult Social Care, and third‑sector partners. The team combines a broad range of skills and expertise in multi‑disciplinary assessments and support services. Together, we take a holistic, assertive outreach approach to support a small caseload of clients across West Sussex who are facing homelessness (or insecure housing), involvement with the criminal justice system, substance misuse, domestic violence, and mental health challenges.
As a Social Worker, your role will be twofold:
* Providing intensive, person‑centred support to help clients lead meaningful, independent lives.
* Undertaking traditional social work duties such as assessments, support planning, and reviews.
A key part of this role is identifying systemic challenges, working flexibly, and drawing on creative solutions and best practices to overcome barriers faced by people with multiple disadvantages. There is a strong emphasis on continuous learning and practice development.
Qualifications
* You will possess the ability to communicate effectively, have compassion and empathy to support people who access our services, and be able to manage your own health and well‑being due to the emotional demands of the role.
* You will have a good theoretical, practical, and procedural knowledge of social work, and a good understanding of the necessity of working with partnership agencies including health, housing, education, and statutory, voluntary and community sector organisations.
* There will also be a requirement for you to work flexibly using your own initiative wherever appropriate. You will be required to travel throughout the county, including to locations that are not always easily accessible by public transport.
* Qualifications and experience: Degree (or recognised equivalent) in Social Work, e.g., Diploma in Social Work; registered with Social Work England; experience of undertaking assessments and social work intervention planning, implementation and review by means of creative and pragmatic solutions.
