Overview
Are you highly organised, adaptable, and ready to make a meaningful difference in community healthcare? We're looking for a motivated individual to join our Urgent Community Response (UCR) and Hospital at home VW team as a Clinical triage support. This is a key role in the service, you will be at the heart of coordinating urgent care for patients in their homes, helping to prevent hospital admissions and support recovery in the community.
Responsibilities
* Monitor shared inboxes and task lists to manage referrals and scheduling visits.
* Support team meetings and maintain accurate, up-to-date records using electronic systems.
* Work closely with clinical teams to support the day‑to‑day running of the service and the continued expansion of Virtual Ward capacity.
* Communicate with families and professionals, ensuring timely updates to clinical systems.
* Adapt to a hybrid working pattern with regular weekly on‑site presence in both localities and weekly travel to both team bases.
Qualifications
* Excellent organisational, IT, and communication skills.
* Ability to manage competing priorities in a fast-paced environment.
* Proactive, adaptable, empathetic, and confident working both independently and collaboratively.
* Experience in a health, social care or community-based role is desirable.
* Full UK driving licence and access to a car is essential.
