Administrator/Care Planner
Location: Sheffield & Rotherham
Salary: £28,000 per annum
Hours: Full time
Caremark Rotherham & Sheffield are seeking a dedicated and compassionate Care Coordinator to join our experienced management team. We proudly deliver high-quality support to children and adults living with complex needs in the community and are rated ‘Good’ by the CQC.
Responsibilities
* Effectively coordinate and maintain Support Workers’ rotas, ensuring continuity of care and appropriate client-staff matching.
* Allocate fully trained staff in line with client-specific care requirements.
* Respond efficiently to telephone and email enquiries.
* Use care planning and rostering systems such as People Planner and Access Care Planning (ACP).
* Liaise with clients, families, healthcare professionals, and colleagues to ensure seamless service delivery.
* Respond to changes in care requirements or scheduling swiftly and professionally.
* Provide regular support and supervision to the care team.
* Participate in the on-call rota as part of a shared team responsibility.
* Hold a valid UK driving license, access to a vehicle, and the ability to obtain business insurance.
Key Requirements
* Previous experience in a care coordination or similar administrative role within health or social care.
* Strong IT skills and proficiency in Outlook, Word, and Excel.
* Ability to prioritise tasks and manage competing demands under pressure.
* Excellent communication and interpersonal skills.
* A strong commitment to person-centred care and high standards of service.
