Due to retirement an exciting opportunity has arisen within our Community Children's Nursing Team in our Eastern District (Burton and Surrounds).
This role will be part of a job-share agreement, working alongside a very experienced Team Leader
The post holder will have responsibility for an established team of several Senior community Children's Nurses, a Family Support Worker and an Administrator, ensuring safe delivery of care alongside operational expectations including budgetary management, sickness/absence management and workforce management (list is not exhaustive).

Main duties of the job
The successful applicant will lead, support and coordinate the provision of the Community Children's Nursing (CCN) service in the East MPFT locality and work with the team and our partners to develop safe and effective Services, reflecting the needs of our population and the expectations of our commissioners.
The service aims to support families in caring for their sick CYP at home, in order to avoid Hospitalisation and/or facilitate early discharge. This is done by the assessment of CYP and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.
The post holder will manage a defined clinical caseload with a range of acute and chronic health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
The post holder together with their job share will have shared responsibility for the team caseload. Ensuring the CYP have their caseloads assessed, developed, evaluated and managed in an individualised manner. This being achieved in partnership with the CYP and family for a range of acute and chronic health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
Direct patient care will remain a part of this job role.
