Job Summary
North Lambeth Primary Care Network is seeking a 1 WTE PCN Care Coordinator to support the Waterloo Health Centre. The role involves working closely with GPs and other clinicians to assist patients in managing their care, providing administrative support across practices, and acting as the first point of contact for patients, clinicians and community carers.
Key Responsibilities
* Undertake work in line with PCN directed priorities.
* Identify and support a cohort of patients, using decision support aids to personalise their care requirements.
* Maintain regular and consistent communication with referrers regarding patient progress, complications and guidance.
* Raise awareness of health promotion and NHS health checks within practices.
* Coordinate clinics, liaise with schedulers and contact identified patients with appointments.
* Manage patient-initiated calls for help or signposting, and book into named GP urgent care or telephone slots as necessary.
* Document, monitor and audit patient coordination activities within the patient administration system.
* Recognise and respond appropriately to sudden patient deterioration or emergencies, alerting the team or enabling a rapid response.
* Support national screening and immunisation programmes.
* Monitor referrals to ensure completion of tasks and timely care delivery, maintaining regular telephone contact.
* Provide direct liaison with multi-agency partners to coordinate patient care.
* Refer patients who may benefit from social prescribing link workers.
* Collect and collate patient and carer feedback on experiences.
* Support quality and outcome frameworks and other DES/LES specifications.
* Maintain engagement with all practice staff, encouraging best practice.
* Act as the first point of call for patients within the caseload related to their care.
* Develop a single, personalised care and support plan (PCSP) for each patient, exploring options to meet identified needs.
* Assist patients with training, employment and access to appropriate benefits.
* Educate patients on shared decision‑making tools and promote preparedness for such conversations.
* Ensure patients receive high‑quality information to make informed care choices.
* Support patients in understanding their health activation levels, including the use of the Patient Activation Measure.
* Facilitate access to self‑management education courses, peer support and interventions that improve health and wellbeing.
* Explore and facilitate access to personal health budgets where appropriate.
* Coordinate and navigate patients and carers across health and care services, working closely with social prescribing link workers and other primary care roles.
* Support the coordination and delivery of multidisciplinary team (MDT) meetings within PCNs, managing meeting logistics and liaison with agencies.
* Liaise directly with care homes and other key providers, identifying patients for MDT discussion and circulating relevant information.
* Other duties as defined by the PCN as necessary.

* Communicates effectively, both verbally and in writing, with patients, carers, partner agencies and stakeholders.
* Negotiates effectively, maintaining productive relationships with all colleagues in the PCN and practice teams.
* Demonstrates personal accountability, emotional resilience and functions well under pressure.
* Knowledge of the personalised care approach and general practice clinical systems (e.g., EMIS ERS).
* Advanced proficiency in MS Word, Excel, PowerPoint and internet tools for creating reports and plans.
* Experience of working with healthcare professionals, NHS or social care settings (including unpaid work).
* Experience of data collection, monitoring information and partnership working across organisations.
* Experience in general practice or multi‑disciplinary settings with influence and negotiation skills.
* Relevant qualifications: ECDL or equivalent, diploma/HNC level or NVQ Level 2 Business Administration, or equivalent experience.
* Clear DBS reference and criminal record status in line with legal requirements.
* Access to own transport, ability to travel regularly across the locality.
* Continued commitment to skills improvement and learning in new areas of work.
