Community Staff Nurse - Ashfields District Nursing Team – Chicago Chiropractic & Sports Injury Centers
An exciting opportunity has arisen for a registered nurse to join our friendly and forward thinking community care team in SMASH. This is a 9 month fixed term post covering secondment. The role requires effective communication and interpersonal skills, adaptability, reliability and approachability, and a sound knowledge of recent changes within the NHS as well as awareness of political drivers to provide care closer to home.
The main base is Sandbach, with work across the CCICP footprint. Travel is an essential part of this role; please demonstrate on your application how you would meet this requirement. Hours are 08:00–17:00, over 7 days.
Responsibilities
* To work under the direction of the Community Nurse Team Leader and be responsible for the assessment and management of health needs of identified populations and for the development, implementation and evaluation of care programmes within a range of target groups, including end of life care and patients with long term conditions and complex nursing needs.
* Support patients in their own place of care where possible, maintain independence and quality of life, hold a designated caseload and deputise in the absence of the District Nurse.
* Liaise with healthcare professionals and other agencies to ensure appropriate and effective service delivery.
* Work within the agreed framework for Community Nursing Services and provide holistic nursing care in the home or appropriate care setting using evidence-based knowledge.
* Undertake initial assessments on receipt of referrals, plan and evaluate a plan of care to meet physical, social, emotional and environmental needs, incorporating rehabilitative/self-care approaches as necessary.
* Provide skilled nursing care and clinical tasks including wound care, leg ulcer assessment and management, administration of medications via syringe pump, intravenous care, venepuncture and catheter care.
* Recognise, interpret and analyse clinical data and demonstrate, teach and supervise nursing care that can be safely delegated to patients/carers.
* Promote clinical excellence and evidence-based care, maintaining clinical competencies in line with the competency framework and adhering to Trust policies.
* Work to the End of Life (EOL) Strategy and contribute to health education of individuals and communities, promoting self-care and planned/opportunistic care based on evidence-based information.
* Accurately record patient contacts on Emis Web and Trust documentation, maintaining confidentiality in line with NMC guidelines.
* Manage discharge potential and needs from the caseload; ensure equipment and aids meet patient needs and are maintained; demonstrate safe use where required.
* Participate in care involving complex needs and support carers and relatives; refer to other practitioners when needs exceed own scope or require longer-term support.
* Prioritise and manage own caseload and client group; assume responsibility in the absence of the Team Leader/Senior Lead within the competency framework.
* Contribute to quality standards measurement and, if applicable, participate in non-medical prescribing in line with local policies and formulary.
* Maintain accurate personal and professional records per MCHFT and NMC guidelines; ensure efficient and cost-effective use of resources.
* Report accidents/incidents on duty and participate in investigations as necessary; coordinate off-duty and holiday leave to maintain service cover; support other team members.
* Maintain all aspects of MCHFT Integrated Governance and participate in appraisal and clinical supervision as per policy; act with privacy, dignity and confidentiality at all times.
