About the Role As a Support Worker (SW), you will play a crucial role in supporting individuals on an individual basis. You will work closely with a team to provide tailored support, helping individuals to overcome barriers and develop essential skills. Responsibilities of Support Worker (SW) Support Team Collaboration: Collaborate with team members to create tailored support plans that address the specific needs of each individual. Regularly review and adjust these plans to ensure they remain effective and relevant. One-on-One Interventions: Offer individual interventions to help individuals improve their outcomes. Use evidence-based strategies to support individuals in developing their skills. Encourage Positive Engagement: Teach and reinforce effective techniques, helping individuals to understand and engage with their daily activities. Encourage positive interactions and model appropriate behaviour. Daily Support: Work alongside team members to implement strategies that promote a positive and inclusive environment. Support team members in addressing challenges and fostering a culture of respect and cooperation. Monitor Progress: Maintain detailed records of individuals’ progress and any interventions used. Provide regular updates to team members, families, and other relevant parties to ensure a coordinated approach to each individual’s development. Liaise with External Agencies: Ensure that individuals receive comprehensive support by collaborating with counsellors, social workers, and other professionals to address the broader needs of individuals and their families. Requirements of Support Worker (SW) Prior experience in supporting individuals is essential. Demonstrate a deep commitment to helping individuals, exhibiting patience and empathy. Ability to establish and maintain strong, supportive relationships with all members of the community. Ability to work effectively as part of a team.
