* Collaborate with multidisciplinary teams including Social Care Assessors, Nurses, Therapists, and the MPFT Home First Team across various hospital and community settings.
* Conduct comprehensive Statutory Social Care Assessments and develop support plans tailored to individual needs.
* Perform Mental Capacity Assessments and facilitate Best Interest Meetings.
* Manage section 42 enquiries in line with the Care Act (2014) and Mental Capacity Act (2015).
* Handle Deputyship Referrals and actively participate in Continuing Healthcare Decision Support Tool Meetings.
* Work independently and as part of a team to effectively support service users.
* Utilize strong communication and IT skills to streamline operations and deliver efficient care.
