Job overview An exciting opportunity has arisen for a motivated and enthusiastic individual to join a newly developing Community Dementia Service as a Dementia Specialist Practitioner within the Complex Dementia Team. The Service is currently being established in the Lincoln & Gainsborough area, with plans to expand into a countywide service in the coming years. The role will involve the assessment, treatment and monitoring of patients with complex dementia presentations to provide support, education and evidence-based interventions to manage complex presentations. As a Specialist Practitioner, the role will also include the clinical supervision and leadership of members of the clinical team, as well as liaising with stakeholders: primary care colleagues, care home teams, and third-sector agencies. Advert The post holder will be responsible for providing high quality, evidence-based care and treatment to residents of Lincolnshire who are experiencing memory problems. The post holder will lead patients and their families / carers through the care journey, including completing initial assessments and cognitive testing, dispersing a diagnosis, initiating treatment and monitoring the individual’s needs, as well as providing professional advice and support to other agencies and members of the multi-disciplinary team. To provide a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools. To work flexibly across a 7-day-a-week service. To delegate aspects of the patient treatment plan to other members of the team, whilst maintaining overall responsibility for patients care, where appropriate. To ensure the clinical caseload and clinical practice of clinical members of the team is of the highest standard of clinical care. To manage caseload and service delivery in accordance with and in order to contribute towards the attainment of Service/Trust productivity requirements. Develop and regularly review care plan and risk assessments Work with individuals to assess their mental health, recognise mental illness and identify their related needs and circumstances; and enable them to understand, manage and where appropriate change their behaviour. Work with individuals who are referred to service with concerns about their memory, and their family / carers, to assess, disperse diagnosis and initiate / monitor treatment, working in line with the divisions agreed pathways. Plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk. To assess carers’ and families’ needs and develop, implement and review programmes of support for cares and families. Protect people from abuse, neglect and harm.
