We are seeking a dynamic, enthusiastic, resilient, and dedicated Clinical Lead Nurse Manager to join our Integrated Nursing and Therapy Urgent Community Response team in North Norfolk. The role involves supporting daily clinical and operational staff management in a fast-paced but rewarding environment. This position is suitable for an experienced nurse with a background in acute, community, or urgent/emergency response teams.
This role offers opportunities for new challenges and ongoing development within a multidisciplinary, innovative, supportive, and friendly team. The team collaborates closely with other community and NHS services to prevent hospital admissions.
Job Purpose
* Provide clinical expertise, leadership, and line management to staff within Urgent Community Response (UCR).
* Support staff in achieving and maintaining competencies within the service.
* Contribute to delivering high standards of healthcare to patients in their own homes.
* Ensure Trust priorities and quality goals are embedded within the workforce by completing annual appraisals.
Hours of Operation
The service operates from 08:00 to 20:00, seven days a week. The successful candidate will work shifts within these hours.
About Us
You will contribute to service and quality development through innovative working methods. The post holder will work alongside the Clinical Operations Manager and UCR nursing and therapy leads to support operational, clinical, and professional staff development across North Norfolk.
The UCR teams provide admission avoidance services via the Rapid Response MDP (formerly NEAT) and 2-hour UCR response processes. They support patients at the end of life and through discharge-to-assess pathways.
Our service participates in current and future innovative projects aimed at reducing hospital admissions and enabling care at home. You will be expected to support Virtual Ward patients and the award-winning Unscheduled Care Coordination Hub.
