Overview
We are looking for an experienced Childrens Mental Health Practitioner to join our Central Basildon PCN for 12 months fixed term.
Context
As part of the NHS long term plan, local areas were asked to realign community mental health services with primary care networks, creating new and integrated models of primary and community mental health care.
Role and responsibilities
* Provide a combined consultation, advice, triage and liaison function in accordance with agreed protocols.
* Support children and young people with mild to moderate mental health problems through evidence-based interventions.
* Assess and manage caseloads of CYP with mild to moderate mental health needs; triage cases, complete full assessments and develop care plans.
* Work with operation manager and Clinical & Strategic Lead to ensure effective delivery of the PCN initiative in Essex.
* Support the development and delivery of the PCN service in Essex; deliver evidence-based low intensity interventions in GP services; liaise between professionals.
* Work with GPs to support children, young people and families in managing presenting difficulties; include onward referrals to secondary services or signposting as appropriate.
* Maintain and manage a clinical caseload with experience in delivering therapeutic and low intensity interventions and meeting service targets and waiting list demands.
Responsibilities (clinical)
* Work as part of a multi-disciplinary team to assess and manage patients using expert knowledge.
* Assess new patients and produce/complete individual care plans.
* Provide specialist clinical support and advice to young people with mental health difficulties.
* Educate and involve family members and others in treatment as necessary.
* Plan day-to-day personal workloads; follow departmental policies and guidelines.
* Triages and receives referrals and directed patients from within the PCN.
* Communicate effectively with patients, using appropriate methods to overcome differences in understanding, culture, and communication preferences.

The post holder will primarily work from GP practices within the Central Basildon PCN and may travel around the county for meetings or training. Areas include:

* Murree Medical Centre
* Langdon Hills Medical Practice
* Fryerns Medical Centre
* Knights Surgery
* Aegis Medical Centre
* Rose Villa Surgery
Additional details
* Probationary period applies (internal applicants may be exempt).
* Certificates of Sponsorship: check eligibility under UKVI points-based system.
* High-Cost Area Supplement: 5% of basic salary (minimum £1,303, maximum £2,198 per annum pro rata).
Requirements
* Experience delivering therapeutic interventions to children and young people with mild to moderate mental health needs.
* Experience in delivering individual and group interventions; caseload management and meeting service targets.
