Were looking for a patient-focused General Practice Nurse with chronic disease management experience, to join our friendly and innovative GP Practice.
Youll play a key clinical role in delivering high-quality, evidence-based care for patients with long-term conditions in our busy, large GP practice. Youll work closely with our lead General Practice Nurse - running your own chronic disease management clinics, working autonomously while collaborating closely with GPs, the wider nursing team, allied health professionals and community services to improve patient outcomes and reduce health inequalities.

What youll do
14. Run nurse-led clinics for patients with chronic conditions - some or all of:
15. Diabetes
16. COPD
17. Asthma
18. Hypertension
19. Coronary Heart Disease
20. Chronic Kidney Disease
21. Undertake comprehensive assessments, diagnosis, care planning, review and evaluation of patients with long-term conditions.
22. Prescribe medications independently (if qualified)
23. Provide lifestyle and self-management education, including smoking cessation, weight management and medication adherence.
24. Monitor, interpret and act upon clinical investigations such as blood tests
25. Identify patients at risk of deterioration and escalate appropriately, including referral to secondary care or community services.
26. Work collaboratively within the multidisciplinary team (MDT), including GPs, pharmacists, physiotherapists, mental health practitioners and district nursing teams.
27. Contribute to proactive care planning for patients with complex needs, including frailty and multimorbidity.
28. Attend and participate in Practice clinical meetings.
29. Ensure clinical practice adheres to national and local guidelines, as well as Practice protocols and policies
30. Contribute to Quality Improvement (QI) initiatives and achievement of GP contract requirements, and participate in audit, service development and evaluation of chronic disease services.
31. Maintain accurate and timely clinical records using Vision and other digital systems, including relevant clinical coding
32. Act as a clinical resource and mentor for other nursing staff and phlebotomists.
33. Maintain NMC registration and professional revalidation.
34. Engage in ongoing CPD related to chronic disease management and primary care.
You must:
35. Be a Registered Nurse (Adult) with current NMC registration
36. Have post-registration experience in primary care and long-term condition management
37. Have qualifications/diplomas in some (but not necessarily all) areas of chronic disease management
38. Cervical screening qualification
39. Show evidence of autonomous clinical practice
40. Have excellent communication and interpersonal skills
41. Have strong IT skills and experience with GP clinical systems
42. Have a patient-centred and compassionate approach to care
43. Be highly organised, with the ability to prioritise a complex workload
44. Be a confident decision-maker with a collaborative working style
45. Be committed to continuous improvement and reducing health inequalities
46. Have existing clearance to work in in the UK

Riverside Medical Practice is based in Musselburgh, close to both Edinburgh city centre and the beaches of East Lothian. Were a busy surgery with 20,000 patients, located in an area with significant health inequality.
This is an exciting time to join our stable, 50-strong team, whose aim is providing the best possible care to each individual patient with the resources we have available to us.
The Practice is based in a purpose-built health centre with recently refurbished staff kitchen, sun terrace and onsite parking. Tea, coffee and biscuits are always available!This is a well-organised practice, with mature systems and processes in place which allow our team to do their jobs to the very best of their abilities.
