Join us in beautiful South Pembrokeshire as a GP Fellow and help shape the future of integrated, community-based healthcare.
This 12-month, innovative GP Fellowship is hosted within the South Pembrokeshire Cluster — a dynamic group of practices, community healthcare professionals, and allied partners. It is designed to support professional development through a blended role involving clinical practice, same day emergency care, hospital at home and dedicated project time focused on improving care for people living with frailty. There is also protected time for self directed learning, mentoring and monthly educational development sessions with the scheme programme director. Financial contribution towards the fees of a higher qualification in a second interest area of £2000. Funding towards the top-up indemnity fee: up to a maximum of £1000

Structured Mentorship and Professional Development: Receive tailored GP mentorship from experienced clinicians across three supportive practices daily, dedicated and protected education time, mentoring time with HEIW and the health board primary care academy.
Project Focus: Work on a cluster-prioritised initiative centred on frailty identification, management, and proactive intervention.
MDT and holistic frailty care focus: offering a unique opportunity to engage in transformational service development. Engage with cluster (Cluster-Led Allied Services Resource) and same-day emergency care, hospital at home, secondary care and frailty consultant.
Leadership & Innovation: Engage with cluster (Cluster-Led Allied Services Resource) and same day emergency care and hospital at home secondary care consultant innovation funding to develop sustainable, scalable models of care.
Health Education and Improvement Wales (HEIW) is the strategic workforce body for NHS Wales with statutory functions that include education and training, workforce planning, workforce development and transformation, leadership and succession planning, and careers.
