Job overview

We're looking for an enthusiastic dedicated nurse who is keen to work in our Frailty (SDEC) Same Day Emergency Care. You will be working in the Frailty Directorate, Medicine Division, at an acute hospital in a care of the elderly setting. This service is focused on delivering rapid assessment; bespoke care to our elderly patients and provide treatments and provision to prevent prolonged stay in hospital and unnecessary hospital admission.

Main duties of the job

The role is varied, equally challenging and rewarding, where you will have the opportunity to really make a difference to the experience of our patients.

The post holder will work in the Frailty Same Day Emergency Care Unit based on C8.The post holder has the responsibility for managing the triage of patient referrals, timely patient interventions and management of admissions and discharges and prioritising/delegating workload.

The post holder will conduct audits related to the professional development of nursing, monitoring standards of care and acting as an agent for change in achieving a culture that is flexible and responds positively to the demands and future development of the service. To undertake a key role in the delivery of the unit’s privacy and dignity agenda. The post holder will be working in an unpredictable environment within the clinical area (e.g. violence and aggression and Clinical emergencies).

The service is currently operating on a 5-day service Monday-Friday but successful candidates will be expected to enter into a 7-day rota when the service develops to a 7-day service.

Detailed job description and main responsibilities

1. Responsible and accountable for the delivery of patient care in the designated specialist area and to influence practice within the MDT.
2. Work on own initiative to undertake a wide range of duties relating to specialist area, backed up by professional skills and knowledge, including being able to respond in the event of an adverse reaction.
3. Be highly skilled and competent in the comprehensive assessment of patients using theoretical knowledge and a wide range of experience to assess needs and administer care to support recovery.
4. Devise individual patient-led treatment plans using clinical reasoning, current evidence and a wide range of treatment skills.
5. Co-ordinate interventions which may include other disciplines through case management, multidisciplinary team meetings, integrated teams within the hospital and in the community and goal setting meetings.
6. Ensure all documentation is complete and accurate and that all information is stored and maintained appropriately by keeping databases and records updated.
7. Plan and implement sessions of care based on sound theoretical knowledge and evidence.
8. Ensure the use of a clinical governance framework and resources to guide practice, participating in supervision and reflection on clinical practice
9. Bring excellent clinical judgemental skills to the analysis and interpretation of a range of clinical options regarding patient care.

This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route however sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds (£25000), and any role-specific criteria.

10. Registered nurse with current UK NMC registration
11. Substantial frailty care experience
12. Competent in venepuncture and cannulation
13. TEACH module or working towards
14. Competent in administering blood transfusions and IV therapy

Desirable criteria

15. Evidence of professional development
16. Evidence of broad clinical skills
17. Leadership courses or proven experience of leadership
