Job overview

We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team, the team deliver responsive high quality nursing to patients living in Shefford and surrounding villages within Mid Beds. We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.

This is an excellent opportunity to develop your knowledge and skills within the community nursing setting. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

Detailed job description and main responsibilities

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

This is a part time (30 hours per week) fixed term post to cover maternity leave.

Education

Essential criteria

1. Registered General Nurse
2. Demonstrate evidence-based practice and up-to-date clinical skills.

Desirable criteria

3. Evidence of post-basic professional development
4. Post-registration qualification in a relevant clinical field
5. Relevant teaching/mentorship qualification or •commitment to undertake
6. Post-registration nursing experience, including community experience

Physical/Mental/Emotional Support

Essential criteria

7. Ability to mobilise and stands for most of shift; and kneels and crouches to deliver clinical care e.g. dress wounds.
8. Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behavior
9. Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
10. Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions

Expereience

Essential criteria

11. Evidence of and commitment to multi- •professional working.
12. Understanding of the interface between primary and secondary care
13. •Comprehensive understanding of the roles of others in health and social care

Desirable criteria

14. Be familiar with SystmOne in order to use the single assessment documentation in achieving person centred care.
15. Be a proactive nurse with a strong desire to lead on the delivery of quality, holistic care

Knowledge

Essential criteria

16. Literate in IT/Computer Skills
17. Evidence of knowledge of clinical governance
18. Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
19. Knowledge of the principles of health promotion and prevention of illness.
20. Good oral communication skills based on fluency in English language

Desirable criteria

21. •Demonstrate an understanding of national government initiatives in health care.

Freedom to act

Essential criteria

22. Work within clearly defined clinical and professional policies
23. Ability to work independently seeking appropriate support and advice as necessary
24. Works within codes of practice and professional guidelines

25. Current UK driving license and access to a car
26. Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.
