The post is part of the specialist urogynaecology nursing service for women with pelvic floor and bladder dysfunction and will work alongside the Urogynaecology Consultant to deliver care according to NICE Clinical Guideline Urinary Incontinence 2013 (CG171).
The post holder will provide specialist clinical expertise in all aspects of care for urogynaecology patients, together with the Urogynaecology Consultant and members of the wider clinical team.
The post holder will effectively co‑ordinate and deliver an expert nursing service within the specialist area and manage their own nurse‑led outpatient service and case load (including vaginal pessary, bladder retraining, patient education/catheter care, Trial without catheter (TWOC), management of prolapse & medication reviews, urodynamic diagnostic clinics & flexible cystoscopies (following appropriate training and assessment) and post‑operative assessment) and, in collaboration with the urogynaecology team, advise on appropriate treatment plans.
The post holder will ensure a positive impact on the experience of patients and carers as well as developing evidence‑based clinical nursing practice in line with current research and guidelines.
This is a fixed‑term / secondment opportunity to cover the existing post holder secondment to another role. Band 6 development post would be considered.
The purpose of the post is to contribute to a specialist urogynaecology nursing service for women with pelvic floor and bladder dysfunction alongside the existing Urogynaecology Nursing staff, Urogynaecology Consultants, and the rest of the multidisciplinary team.
Responsibilities
* Triage all incoming referrals to the service.
* Provide specialist nursing advice and support to patients, families, carers and other healthcare professionals following diagnosis and through treatment.
* Ensure continuity of high‑quality evidence‑based nursing care, including assessing the health and nursing needs of patients, families and carers.
* Identify and initiate appropriate effective care, including managing a patient caseload, ordering and performing diagnostic tests, making referrals to appropriate specialities for onward treatment as required, and admitting and discharging women with continence needs within agreed protocols.
* Undertake outpatient clinics for nurse‑led pessary, urodynamic diagnostics, pelvic floor and incontinence clinic, support group and post‑operative follow‑up clinic.
* Ensure the BSUG database is kept up to date to enable accreditation of the service.
* Work within UKCS Pessary Guidelines and urodynamic standards.
* Accept responsibility for own patient caseload, ensuring all patients have an accurate plan of care that reflects the assessment undertaken and incorporates issues and recommendations made, ensuring clear documentation in the patient’s records, including outcome letters sent to patients’ GP surgery.
* Perform urodynamic studies and potentially other invasive procedures (such as flexible cystoscopies) independently and in collaboration with the urogynaecology clinicians, advising on appropriate treatment plans.
* Actively involve in inpatient care, liaising with ward staff to ensure high‑quality nursing care including support of timely discharge of post‑op patients and ensuring follow‑up care to appropriate urogynae clinic is arranged.
* Follow up patients as appropriate and ensure all relevant information is available to enable patients to make an informed choice about their treatment.
* Attend, coordinate and contribute to multidisciplinary team meetings, ensuring all relevant patients are represented.
* Deliver a seamless service through the development of enhanced multidisciplinary team processes and communication.
* Participate in the identification and development of clinical protocols and strategies to enhance both the continuity and standard of urogynaecological care whilst ensuring equity of access to the service.
* Demonstrate clinical skills and expertise to all grades of staff, including junior medical staff; review and evaluate care given by other team members, ensuring that realistic goals are set to achieve optimal outcome.
* Act as a role model and clinical leader, demonstrating knowledge and research‑based best practice.
* Identify safeguarding and vulnerable adult issues which affect the rights of individuals.
* Develop appropriate clinical skills related to specialist practice such as pessary insertion, urodynamics, continence assessment, and menopause assessment.
* Work with key staff members to deliver nurse‑led assessment and outpatient follow‑up services, providing detailed baseline patient assessment and education and support to patients and their families.
* Work with the multidisciplinary team to develop, implement and evaluate integrated care pathways and systems of multidisciplinary team documentation.
* Liaise with local community, social services and voluntary organisations to develop existing services for clients, carers and health professionals.
* Involve service users actively in providing feedback on their experience of the current service and suggestions for improvements.
* Participate in the development of appropriate patient group directives to facilitate and enhance the discharge process.
* Ensure all patients are fully informed of the risks and benefits of proposed procedures and treatments, contributing to the process of securing informed consent.
* Produce and provide relevant and accessible written information regarding treatment to all patients in accordance with Trust standards.
* Prescribe safely and appropriately in line with the NMP formulary (if held).
