Our cohesive team comprisesDoctors, highly skilled Nurses, Healthcare Support Workers, a Prescribing Team,Care Navigators, and an Administration Team, all committed to deliveringhigh-quality, patient-centred healthcare. We strive to combine clinical excellencewith innovation and collaborative working to meet the needs of our patients andcommunity.

In addition, we activelysupport the vocational training of new General Practitioners and MedicalStudents through the GP Training Programme and the Wales Academy for Learningin Primary Care, contributing to the development of the future primary care workforce


Main duties of the job

Hold clinics for patients requiringface-to face or telephone Structured Medication Reviews (SMRs), prescribing,monitoring, and optimising medicines to manage long-term conditions.

Contribute pharmaceutical advice forthe development and implementation of new services/systems that havemedicines/prescribing components (e.g. Chronic Disease Management, QI projects,LES).

Contribute to public healthcampaigns.

Undertake changes to medicines(switches) designed to save on medicine costs in accordance with localformularies, or in response to disruptions in the medicines supply chain.

Answer medicine-related enquiries orrequests from GPs, other practice staff, and patients.

Analyse, interpret and presentmedicines data to highlight issues and risks to support decision making.

Undertake audits of prescribing,feedback the results and implement changes in conjunction with the practiceteam.

Monitor practice prescribing againstthe local health board formulary and make recommendations to GPs for medicinesthat should be prescribed by hospitals only or subject to shared carearrangements.

Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in care homes).

Undertake any other duties appropriate to the grade.

Participate and support personal development reviews and objective setting with your clinical line manager.

We offer a range of NHS services in the Practice via our multidisciplinary and highly skilled team. We currently have 5 GP partners, four Salaried GPs, 2 registrars (Trainee GPs), two Practice Nurses, 2 Health Care Support Workers, cluster social prescribers and physiotherapists.

We benefit from a clear and effectivemanagement structure,led by a highly skilled and experienced Practice Manager who provides strongoperational leadership and oversight. The Practice Manager is supported by adynamic and knowledgeable care navigation and prescribingteam, ensuring efficient coordination of patient services and day-to-daypractice operations.

This collaborative approach helps us deliver high-quality, patient-centred care while fostering a supportive and inclusive working environment.


Job responsibilities

Key Responsibilities

1. Chronic Disease Management

* Manage a caseload of patients with all chronic conditions, including complex polypharmacy cases and long-term conditions such as diabetes, hypertension, asthma, and COPD.
* 2. Clinical Medication Reviews
* Conduct comprehensive medication reviews for patients with multi-morbidity, focusing on optimising treatment plans and aligning with individual care goals. Specifically focussing on poly pharmacy and prudent prescribing initiatives.
* 3. Medicines Management in Care Homes
* Undertake systematic medication reviews for care home residents, addressing polypharmacy and ensuring the appropriateness of each medication (When enhanced service is implemented).
* 4. Leadership in Medicines Optimisation
* Provide clinical leadership on medicines optimization strategies, utilising audit metrics to identify areas for improvement in prescribing practices.
* Run patient-facing clinics to address medication concerns, provide education, and support adherence to prescribed treatments.
* Manage a telephone helpline for patients with questions about their medications, ensuring timely and accurate responses.

6. Unplanned Hospital Admissions

* Identify high-risk patients through risk stratification tools and implement strategies to reduce preventable hospital admissions.

7. Quality Improvement Initiatives

* Lead quality improvement programs, including clinical audits to assess prescribing practices against national guidelines or local formularies.
* Analyse prescribing data to identify trends, risks, and areas for improvement, presenting findings to the practice team.

8. Education and Training

* Deliver targeted training to the prescribing team on topics such as appointment triage, medicines optimisation, and protocol adherence.
* Mentor junior pharmacists (as applicable), the prescribing team and other healthcare professionals, sharing expertise and fostering a collaborative learning environment.

9. Mentorship and Team Development

* Act as a mentor to prescribing team members, providing guidance on appointment allocation, time management, and workload prioritisation.
* Pair with team members for practical case discussions, enabling shared learning and consistency in care delivery.
* Facilitate regular review sessions to reflect on challenging cases and suggest improvements to clinical protocols.

10. Refining Appointment Protocols

* Work with prescribing teams and practice management to develop clearer criteria for when cases should be allocated to the pharmacist.
* Participate in a pilot program to test revised appointment protocols, providing feedback to refine and optimise processes.
* Support efficient workload distribution to ensure that clinical expertise is utilised effectively.

11. Feedback Mechanisms

* Establish a structured feedback loop to report and address inappropriate bookings, offering alternative management strategies where applicable.
* Share insights on problematic patterns in patient referrals during team meetings to enhance collaborative learning and refine practice protocols.

12. Public Health and Preventive Care

* Support public health campaigns by providing expertise on vaccination programs, smoking cessation, and other preventive healthcare measures.
* Promote awareness of local and national initiatives aimed at improving population health outcomes.

13. Systems Development and Data Management

* Utilise clinical software to design and manage searches for at-risk patient cohorts, ensuring timely intervention for medication-related risks.
* Monitor prescribing data against local formularies and NICE guidance, ensuring compliance and suggesting improvements.
* Develop dashboards for tracking consultation durations, medication adherence rates, and prescribing outcomes to inform practice decisions.

Collaboration and Communication

* Foster effective relationships within the practice and across the wider healthcare system, including community and hospital pharmacy teams, local health boards, and other allied healthcare providers.
* Actively participate in multidisciplinary team meetings, contributing to patient care discussions and planning.
* Liaise with community pharmacists, dieticians, and other professionals to ensure seamless care coordination and medication support.

Knowledge, Skills, and Experience

Essential Qualifications:

* Minimum of five years post-graduate experience in pharmacy.
* Independent prescribing qualification or evidence of working towards this qualification.
* Strong understanding of primary care systems, NICE guidelines, and local health board formularies.

Skills and Competencies:

* Proven ability to manage complex medication regimens, balancing efficacy, safety, and cost-effectiveness.
* Strong analytical skills to interpret prescribing data and audit results, using these insights to drive improvement.
* Excellent communication skills for patient education and professional collaboration.
* Experience in delivering training and mentoring to multidisciplinary teams.

* Demonstrate a proactive approach to quality improvement and innovation within primary care.

Special Working Conditions

* The role may involve travel between practice sites and attendance at external meetings.
* Some contact with body fluids may occur during clinical practice.

Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to:

Patients, GP, nurses and other practice staff and other healthcare professionals including LHB pharmacists, pharmacy technicians, optometrists, dentists, health and social care teams and dieticians etc.

Person Specification


Qualifications

* At least 2 years post qualification experience as pharmacist
* Master Degree in Pharmacy or Equivalent
* Registered with GPhC
* Post qualification experience of working in a primary care environment
* Member of Royal Pharmaceutical Society
* Independent non medical prescriber
* Additional qualifications in substance misuse / anticoagulation management
* Diploma in Clinical Pharmacy or equivalent experience in assessment and management of Long Term Condition


Experience
