Are you an experienced Registered Nurse or Paramedic looking for your next challenge?
We have an opportunity to join our team at the Rapid Access Care Unit (RACU) based at Townlands Hospital, Henley-On-Thames.
RACU is a Community Same Day Emergency Care (SDEC) unit which provides sub-acute care for medically unwell patients over the age of 18 years. Each patient receives an individualised MDT assessment, diagnostics and treatment, with a commitment to patient-centred care. The Team comprises doctors, nurses, paramedics, physiotherapy and occupational therapy.
This post would suit a registered nurse or paramedic from either an acute background or community setting. We need a wide range of skills for our patient group, and support and development is available to achieve this.
RACU is part of the Intensive Community Care (ICC) Pathway and works closely with our other SDECs in Witney and Abingdon, Hospital at Home and Urgent Community Response to provide care for patients that is closer to home, with aim to prevent any unnecessary hospital admissions.
The Unit opening times are currently 8am-6pm Monday-Friday.
Responsibilities
* Directly care for patients, conducting a thorough patient assessment and discuss with medical team regarding care planning and treatment.
* Conduct home visits as required to meet the needs of the patient and the service.
* Work within the MDT to improve patient outcomes, making appropriate referrals to other services as required.
* Caring for the acutely unwell patients and their families, including those at the End of Life.

Qualifications & Experience
Essential criteria
* Qualification of Registered Nurse / Paramedic Degree / Diploma
* Current NMC / HCPC Registration
Desirable criteria
* Post Graduate Certificate
Experience
* Essential: Relevant NHS experience as qualified clinician
