CPFT are delighted to offer you the unique opportunity to be part of a Home Treatment Team to support 12-18yr old young people and their families/carers across Cambridgeshire and Peterborough.

This is an exciting opportunity to be involved with a multi-disciplinary team that offers the highest quality service provision for service users & their families/carers.

We are looking for qualified practitioners who are passionate, dynamic, & skilled professionals & we would support staff who are ready for a new or different challenge to significantly improve young people’s experiences.

We can offer a bespoke introduction and training in Children’s and Young People’s Mental Health Services that will enhance your knowledge and skills in such interventions as risk assessments of young people.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

Pathway 1: To offer support to young people and families at a time of crisis, when an admission to hospital may be considered, and with the hope that this could be prevented.

Pathway 2: To support the engagement of complex young people with CAMHS services. To supplement the local CAMHS provision to support the above and to support the management of risk within the community for these young people and their families.

• This is a team which will provide a high standard of safe, effective and responsive care to those young people and their families, experiencing crises or needing intensive home treatment as an alternative to hospital admission.
• You may be required to in-reach to the T4 units during an admission to help shorten a stay and assist our service users and their families to manage in the community as soon as safe to do so, as part of a plan of discharge.
• The service users and their families are the experts regarding their family life and the norm for them, as a service we must respect and value their views. We must enable, and help them build resilience.
• To provide support programmes for young people and their carers, identifying needs and providing therapeutic intervention, utilising the most appropriate method to meet identified needs.
• Take part in the evaluation of the care to individual service users at review meetings, case conferences, and clinical meetings etc.
• Assessing needs and implementing the care planned with service users, their families along with other staff members within the Care Programme Approach.
• To ensure the team provides appropriate home treatment interventions and supports the reduction of admissions to adolescent in-patient beds.
• To manage a caseload, covering the county, so the ability to travel between bases is essential.
• Provide & receive regular clinical supervision in accordance with good practice guidelines and Trust policy.
• To comply with PALS and Complaints policy.
