The North Manchester Early Intervention for Psychosis Team offers a full‑time permanent Care Coordinator post. Registered nurses, social workers or occupational therapists are welcomed to apply.
The service is divided into three teams across the city. This role is based in the North team (Central Park, Northampton Road) and will travel to the city and Greater Manchester inpatient units as part of the back‑up assessment practitioner rota.
You will work with experienced colleagues and be supported by the Clinical Team Manager and Senior Practitioner.
Position: full time (37.5 hours per week). The team operates Monday‑Friday 9 am to 5 pm; flexible working can be discussed.
Responsibilities
* Oversee the clinical management of a caseload of service users aged 14‑65.
* Complete Social Care Act assessments and attend the Funding Panel.
* Participate in duty rota: service‑wide triage worker, back‑up assessment practitioner for service‑wide assessments, back‑up worker for the North team.
* Encourage and support clients to collaborate in their treatment programme and recovery.
* Promote self‑help, maintaining skills and independence in clients.
* Support health promotion and act as a consultative adviser to other community services/agencies regarding clients’ mental health issues.
* Promote a culture of learning, education and professional development – the team often hosts student nurses and social workers on placement.
* Adhere to safeguarding principles for children and vulnerable adults in line with Trust policy and local reporting procedures.
* Participate in safeguarding S42 investigations.
* Undertake specialist mental health assessments in line with the Mental Capacity Act 2007, the Mental Health Act and local safeguarding adults procedures.
* Undertake and record comprehensive risk assessments, devise and implement risk‑management plans for clients at risk of vulnerability, self‑harm or harm to others using the Risk Assessment Tool.
