Job Description
We are looking for a skilled and proactive Clinical Pharmacist (Band 7) to join our team. You’ll play a vital role in ensuring the safe and effective use of medicines through ward-based services, medicines reconciliation, discharge planning, and participation in multi-professional team meetings. This is an exciting opportunity for a pharmacist committed to delivering high-quality, patient-focused care across the system.Key Responsibilities
* Screening medication charts, discharge prescriptions, and one-stop orders to ensure safety and effectiveness
* Medicines reconciliation within 24 hours of admission and at discharge
* Interpretation of blood results in relation to medicines and adjusting therapy accordingly
* Respond to ward-based medicines information (MI) queries
* Attend and contribute to MDTs and consultant ward rounds
* Check controlled drugs (CDs) and perform regular ward audits
* Order medicines and liaise closely with Medicines Optimisation Technicians
* Support the safe transfer of patients across care settings, ensuring complete and accurate discharge summaries
* Contribute to antimicrobial stewardship and public health campaigns (e.g. flu vaccinations)
Skills and Systems Experience (Desirable)
* Working knowledge of SystmOne (S1), SeeEHR, ICM, and pharmacy dispensing systems
* Proficient in Microsoft Office applications
* Ability to handle complex medicines queries using clinical databases, protocols, and national guidelines
Core Competencies and Expectations
* Apply clinical reasoning and judgement to assess and tailor medication for individual patients
* Recommend therapies even where guidance may be limited or controversial
* Identify and report adverse drug reactions via the Yellow Card system
* Use risk stratification tools and implement changes following national drug alerts
* Provide education and training to healthcare professionals and students
* Contribute to clinical audits, policy development, and service improvement initiatives
* Work with autonomy and flexibility to support cover across different wards or services during absence
* Participate in CPD sessions, peer reviews, and maintain professional registration through revalidation
* Support the delivery of NHS Devon ICB’s Medicines Optimisation agenda in primary care
Qualifications and Experience
Essential:
* MPharm degree (or equivalent)
* Registration with the General Pharmaceutical Council (GPhC)
* Practical training certificates and DBS with update service
* Evidence of active CPD and commitment to further development
* Minimum 18 months post-qualification experience in hospital, primary care, or community pharmacy
* Understanding of multidisciplinary team (MDT) roles and care settings
Desirable:
* Working towards or holding a Clinical Diploma and/or Independent Prescriber qualification
* Previous experience of ward-based clinical pharmacy in an acute or community setting
