Job summary: An opportunity for a dynamic and self-motivated nurse to join the chronic kidney disease (CKD) team. This role involves working within an established multidisciplinary team to provide support to patients and their families across Devon and Cornwall, with responsibilities in both outpatient and inpatient settings. A full driving license and access to a car are essential. The role emphasizes education and health promotion in line with regional and national strategies, with a focus on advanced practice within CKD/renal care.
Key duties include managing the CKD team in the absence of the Band 7, ensuring cross-cover for patient caseloads, completing appraisals and sickness management for junior staff, conducting nurse-led CKD clinics on site and at peripheral hospitals, and supporting transplant-related on-call activities. Home visits may be required. The post includes an orientation programme and on-call duties from home (prospective 3 in a six-week period once established).

* Salary: £38,682 to £46,580 per year (pro rata if part-time)

Job Responsibilities
* Ensure CKD patients starting renal replacement therapy receive appropriate education to make informed choices about their options.
* Manage the CKD team in the absence of the manager and coordinate CKD patient care with renal anaemia management.
* Implement individual education programmes for patients and carers entering the CKD pathway.
* Promote health education and ongoing development of CKD patient care, including End of Life (EOL) planning where applicable.
* Maintain up-to-date knowledge and contribute to nursing research and clinical audit in CKD/renal care.
* Identify potential renal transplant recipients and participate in the transplant on-call rota.
* Work collaboratively with the multidisciplinary team and support induction of new staff to renal services.
* Undertake home visits when required.
Qualifications and Experience
* Essential: NMC Registration; post-registration specialist qualification or equivalent where appropriate.
* Essential: Demonstrable experience in a similar renal/CKD role; good communication and influencing skills.
* Desirable: Leadership experience; ability to facilitate change within the work area.
