Overview
We are looking for an experienced band 6 Physiotherapist to deliver high quality care within patients' homes. This is a permanent part-time contract for 30 hours per week, with pattern of work to be negotiated. The post is co-located with our nursing and social care colleagues and involves working within a knowledgeable multi-disciplinary team. We provide in-service training, mandatory and personal development training, 1:1 sessions, and clinical rolling skills training via practice development facilitators. We also host students on training and maintain close working relationships with neighbouring community teams in Health and Social Care.
Main duties and responsibilities
* Provide high standard patient care in patients' homes.
* Facilitate a seamless flow for patient care between hospital and community settings, enabling care in the patient\'s home environment.
* Provide supervision and support to more junior staff.
* Triages referrals to the community team.

Person specification
Experience
* Experience of supervising staff and students
* Experience of participating in research and audit
* Evidence of skill relevant to rehabilitation of adults with physical disabilities
* Experience of working with adults with complex physical, sensory and cognitive disabilities
* Experience with a range of primary and community services including Health, Social Care and the independent sector
* Proven post-registration experience within a multi-disciplinary service
* Leadership experience in a health/social care setting
Desirable
* Current professional student practice placement educator training
* Membership of a relevant professional group
Qualifications
* Diploma or Degree in Occupational Therapy or Physiotherapy
* Health Professions Council Registration
* Membership of an appropriate professional body (desirable)
Knowledge
* Continuing Professional Development and clinical governance awareness
* Health & safety legislation and moving and handling of dependent adults and older people
* Knowledge of NHS plans, NICE guidelines, and related service implications; ability to develop and maintain stakeholder relationships
* Professional and ethical understanding; ability to communicate effectively in writing and verbally in English
Skills
* Willingness and ability to travel across East Suffolk using own transport (UK valid licence; pool vehicle may be available)
