Care Community Administrator (Internal Only)

CHAW Integrated Community Nursing and Therapy Team are looking for a proactive and enthusiastic individual to provide administrative and secretarial support to the multi‑disciplinary team. The role is based from Handforth Health Centre, working Monday and Tuesday 8.30am – 4.30pm as part of a job share. The Integrated Community Nursing and Therapy team provide nursing and therapy interventions and care to patients in their own homes or other community locations for the Chelford, Handforth, Alderley Edge and Wilmslow localities. The post holder will often be the first point of contact for patients, their caregivers or clinicians. Excellent verbal and written communication skills, computer literacy and an ability to handle sensitive information are essential.
Job Responsibilities
* Provide administrative and secretarial support for the integrated community nursing and therapy care community team.
* Act as the first point of contact for patients and clinicians.
* Support day‑to‑day operational functions of the teams and undertake office duties to maintain efficiency of the administrative service.
* Perform a variety of tasks to support the patient journey from referral to consultation to discharge.
* Provide an efficient and effective typing and clerical service.
* Use computerised systems for ordering consumables, booking and tracking community appointments and patient records, ensuring information is coordinated, accurate and up to date.
* Provide a reasonable level of cover for colleagues in other integrated teams in the event of absence.
Qualifications
Essential
* 5 GCSE's or equivalent, grade C or above, including English.
* 3 A Levels or equivalent qualifications at Level 3, or equivalent.
* Typing qualifications to RSA (Royal Society of Arts) 1 standard.
* Computer literate to ECDL (European Computer Driving Licence) standard or equivalent.
Desirable
* AMSPAR Diploma (The Association of Medical Secretaries, Practice Managers, Administrators and Receptionists).
Knowledge & Skills
Essential
* Computer literacy.
* Awareness of current NHS Priorities and targets.
* Good communication skills, able to deal with people at all levels.
* Ability to handle sensitive information without compromising confidentiality.
Desirable
* Medical terminology.
* Knowledge of NHS Computer Systems and experience with Patient Management System and Emis system.
* Awareness of NHS policies.
Experience
