Overview
We have an exciting opportunity for a Senior Primary Care Nurse to join our Primary Care team at HMP Grendon Hill & Spring Hill located in North Buckinghamshire. This is a Cat B Trainer site based on a group therapy model. Grendon is a category B, male Prison and Spring Hill is a Cat D, male and resettlement function. You would work across both sites. Parking on site, access to gym, and would be difficult to get there by bus due to the shifts. Friendly working culture with good relationships with prison staff. Good scope for career development within the role.
Working Hours and Pattern
Hours 37.5 hours - 2 week rolling shift patterns, 7.30am and latest finish at 7pm, working Monday to Friday. Usually working 3 to 4 long days each week. Working one in two weekends from 7.45am-5pm. No night working.
Responsibilities
* Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions.
* Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
* Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
* Deliver non-judgemental care that makes a real difference.
* Inspire excellence by forging strong links with our partners and stakeholders and providing a service that people can trust, feel safe within, and feel proud of.
* Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.

Qualifications and Experience
* Registered nurse Adult
* Primary Care Substance Misuse experience - Desirable or a willingness/interest to learn.
* Current professional qualification (NMC).
* Long terms conditions experience - Desirable.
* Evidence of Continued Professional Development.
* Understanding of the complexities of multi-disciplinary and multi-agency working.
